So is she gonna remain “eyeless” through most of the story or will some of the transformations show it?

She'll remain eyeless most of the time, but her eyes will still be present under her hair, allowing for them to occasionally appear. Technically though, they're false eyes since mimic slimes have no eyes.

Based on what I’ve heard, this is the second best of the trilogy, right? Dark Souls 2 was the more contentious one in the series.
